Swing Bed Program


HOW IT WORKS


Swing Bed Care at Massac Memorial Hospital (MMH) allows you to continue your recovery while staying close to home in a community that understands your needs. Our program is designed entirely around you with your safety in mind.

To qualify for the Swing Bed Program, your physician must identify that you have a skilled need for continued care. Your doctor may initiate a referral to our Swing Bed Program where our team of highly skilled case managers, therapists, and swing bed medical director will review if MMH is the best place for your continued care. Depending on your needs, you could be with us for one to three weeks.

ADVANTAGES OF SWING BED CARE AT MASSAC MEMORIAL HOSPITAL


BRIDGE THE GAP FROM HOSPITAL TO HOME:
Swing Bed Care is for that in-between moment when patients are discharged from the hospital but are not quite ready for the demands of their home environment. We provide patients with the opportunity to reach their optimal level of recovery. Our Swing Bed Program will help you regain your independence, identifying the tools you need for a safe return home.

24 HOUR NURSING CARE:
Our staff is devoted to providing care focused on your safety and recovery so that you can return home in your best condition. We encourage free flow of information and education between our patients and caregivers.

INTERDISCIPLINARY CARE TEAM MEETINGS:
Your physician, nursing, therapy, and case management team will meet routinely to customize and update.

At Massac Memorial Hospital, we consider each patient's individual situation and offer rehabilitative therapies unique to each person. Simple activities of daily living such as dressing, bathing and eating may be challenging during recovery. We teach our patients energy-conserving and alternative methods to complete these tasks.

PHYSICIAN ACCESS:
Your physician will see you, ensuring your thoughts and concerns are heard. Any day in between, the nursing staff are able to bridge any gap in communication; advocating for you with your physician.

FOOD FOR THE SOUL:
An appropriate well-rounded diet is essential to the healing process. We have a dietician available to customize a diet based on your individual healing needs. Our patients rave over our homemade meals and desserts (often asking for seconds).

PRIVATE ROOMS:
The security and privacy our patients is of upmost importance at MMH. We strive to provide all patients with private rooms including dedicated en-suite bathrooms, and a wardrobe for storing personal belongings.

Massac Memorial Hospital Swing Bed Program: A Patient's Journey


At age 72, Paul Greene was feeling anything but his age. He was constantly on the go visiting church family, fishing, and lake living. Some would say his schedule would challenge someone 30 years and younger.

Summer of 2020, Paul became extremely dizzy and unable to walk unassisted and was admitted to Baptist Health Paducah for evaluation. It was later determined that he had a minor stroke and required additional rehabilitation care once discharged. Paul was highly encouraged to participate in Massac Memorial Hospital's Swing Bed Program to regain the strength needed to get back to his lifestyle. As well, allowing him to be a short distance from family.

Recently graduating from Massac Memorial Swing Bed Program, Paul is ready to get back to his busy lifestyle of visiting church family, fishing, and lake living. Paul credits the swing bed services at Massac Memorial Hospital for his recovery.

All other physicians/providers on staff at Massac Memorial Hospital, including but not limited to; emergency physicians/providers, pathologists, radiologists, anesthesia providers, other hospital-based physicians/providers, my attending physician/provider and consulting physicians/providers are independent contractors and are NOT employees or agents of Massac Memorial Hospital. All such physicians/providers are independent contractors who have been granted privileges at this facility for the care and treatment of their patients.
